Printable Guitar Repair Forms

The DVD with the new
Guitar Player Repair Guide
includes printable repair forms.
If your computer doesn’t read DVD discs, no problem: here are all of those PDF files, downloadable for free.

Use the acoustic guitar drawing for taking notes when you inspect a guitar. Circle loose braces, put arrows to show where cracks are, etc. For professional shops, this is a help in describing trouble spots to a customer. Don’t worry about differences between this drawing and the guitar you’re working with; think of this as a starting point for you to draw on. (Based on a Martin dreadnought.)

Use this simplified Strat drawing the same way: as an approximate electric guitar shape to make notes on.

Galloup Guitars was nice enough to let us include this form they use when taking in a repair job. It provides a place to record all of the needed information about the customer and the guitar, and a place to list the proposed work. Be sure to get the customer’s signature on the estimate!

Another Galloup Guitars form. Print these claim tags on card stock and give one to the customer as a record of the work and price agreed to up front. Customers tend to keep these cards as a record of the repair for years—better than a business card.
